Enrichment Class Proposal Form

    Please fill out this proposal completely and as accurately as possible. Incomplete proposals will not get approved.

  4. Class Size

    What is the minimum and maximum number of participants needed to run the class successfully?

  5. Keep the description to 65 words or less.

  6. (What participants should bring, what to wear, etc.)

  8. Typically, Enrichment classes are ran for a month at a time. So, 4 weeks consecutively.

  9. Please put start time and end time.

  11. Class Cost Analysis
  12. Please list the amount you expect to make per hour/per participant

  13. Please list the cost of any supplies/uniforms participants will purchase from you (Ex: If you spend $100 for a class for 10 participants, your supply fee should be $10 to cover the cost)

  14. The supply fee must be a direct cost for the items that participants are receiving. Please be prepared to provide staff with proof of cost for items. 

  15. Draw out exactly how your class set-up should look. Also please provide the exact number of chairs and tables needed. Along with the types of tables (round or rectangle), and if you need any other equipment (laptop, projector...)
